Dr Abbas Al-wasffi has specialised in Obstetrics and Gynaecology for over 13 years, he is Fellow of the Royal Australian and New Zealand College of Obstetricians & Gynaecologists (FRANZCOG). Dr Al-wasffi has a gentle and kind demeanour and compassionately cares for women with gynaecological concerns or at any stage of their pregnancy.


Dr Al-wasffi completed his training at Campbelltown & Liverpool Hospitals, as well completed his fellowship in advanced laparoscopic and pelvic surgery at Campbelltown & Bankstown Hospitals. He is also involved in teaching medical students from the Sydney south West Medical School.


Married with two children, Dr Al-wasffi appreciates the joys of parenthood along with the anxieties around pregnancy and childbirth. Having been involved in the management of many pregnancies, he has developed the skills required to minimise risk and give the assurance to expecting patients. He works with women and their partners to facilitate a successful and enjoyable birth process. He is a conjoint lecturer at the School of Medicine, Western Sydney University. Dr Al-wasffi is available at his consultation room in Campbelltown and Fairfield, He is a consultant at Campbelltown & Fairfield Public Hospitals as well as Sydney South west Private & Campbelltown private hospitals.

Abstract

(For the purpose of this discussion, morbid obesity will defined as body mass index of greater than 35 Kg/M2)

The prevalence of obesity among Australians has risen dramatically, and a reported 35% of Australian women between the ages of 25 and 35 are either overweight or obese. Obesity in pregnancy is associated with an increased risk of maternal, peripartum, and neonatal complications. However, specific guidelines for the management of obese pregnancies are lacking. We hypothesized that induction of obese women at term would decrease the rate of maternal and foetal complications. The aim of this study was to evaluate the prenatal and maternal outcomes of pregnancies in obese women whose labour is induced at term (37 to 40 weeks gestation) versus the standard management of induction post-term (41 weeks to 41 weeks + 10 days). Our data suggest that early induction at term reduces the risk of failed induction (OR=0.41, CI=0.21-0.78) and caesarean delivery (OR=0.46, CI=0.25-0.83), reduces the total duration of labour (p<0.0001), and reduces the length of hospital stay (p<0.0001).


Conclusion

Induction of labour at term in morbidly obese women with BMI > 35 kg/m2 reduces the rate of epidural failure, the rate of delivery by Caesarean section, the total duration of labour, the length of hospital stay, and the likelihood of delivering a large infant, compared with induction post-term. However, early induction did not have a significant effect on other maternal and foetal outcomes. Given these favourable outcomes and the associated reduction in the use of hospital resources we propose that induction at term should be considered in obese pregnancies although a larger study is required to confirm the results of our study
